Immigration Medical Exams (I-693 Form)

Dr. Lee is a certified Designated Civil Surgeon, licensed by the United States Citizenship and Immigration Services (USCIS) to perform your immigration exam, do all the necessary testing, update your vaccine requirements, and complete your I-693 Form — all in the same visit.

What to Bring

• Passport or Driver's License
• Any medical records you may have, especially past vaccine records (no need to worry if you have none; photos are NOT needed)

DOT / Coast Guard Physical Medical Exam

Dr. Lee is a Certified DOT Medical Examiner, licensed to provide DOT Medical Cards for CDL and non-CDL drivers. He also performs Coast Guard Exams including the CG 719K (KE) Forms. The clinic serves as a collection site for many companies with CDL drivers and marine employees.

Travel Vaccines

Dr. Lee is Board Certified by the International Society of Travel Medicine, specializing in Yellow Fever and other travel vaccines. Whether you are traveling internationally for business or leisure, we can ensure you have the required and recommended immunizations for your destination.

Drug Testing

Dr. Lee is a Certified MRO (Medical Review Officer), licensed to perform and interpret SAMHSA lab-approved drug tests for U.S. Coast Guard Marine License holders and all mandated DOT drivers and operators. The clinic serves as a collection site for many companies with CDL drivers and marine employees.

Pre-Op, Work & Sports Physicals

We do pre-op, work, and sports physicals of all kinds. No appointment is ever needed. Bring the form your specialist, employer, or school has provided. We can also do labs and administer any required vaccines in the same visit.

Opiate Use Disorder Treatment

Dr. Lee is a Board Certified Addiction Medicine Specialist and licensed Suboxone prescriber. Our program is streamlined toward individual needs, with heavy emphasis on exercise and counseling. No appointment needed for the initial visit — we may have a waiting list before beginning your evaluation.
