Immigration Medical Exams (I-693 Form)
One Stop Visit. Dr. Lee is a Certified Designated Civil Surgeon licensed by USCIS to perform your immigration exam, complete all required testing, update vaccine requirements, and complete your I-693 Form — all in the same visit.
DOT & Coast Guard Physicals
Certified DOT Medical Examiner providing DOT Medical Cards for CDL and non-CDL drivers. Also performs Coast Guard Exams including CG 719K (KE) Forms.
Travel Vaccines
Board Certified by the International Society of Travel Medicine, specializing in Yellow Fever and other travel vaccines.
Drug Testing
Certified MRO (Medical Review Officer) licensed to perform SAMHSA lab-approved drug testing for U.S. Coast Guard Marine License and all DOT-mandated drivers and operators.
Suboxone Treatment
Board Certified in Addiction Medicine and a licensed Suboxone Prescriber.
Physicals
Walk-in physicals for pre-op patients, employers, and schools — no appointment needed.
